You can set up the integration for Zoho Survey under Setup > Apps & Add-ons > Zoho Survey. There are no pre-requisites for setting up the integration.

Click Enable Zoho Survey to set up the integration. After the integration is enabled, you can disable or configure the integration by clicking the respective button. You can navigate to Setup > User Survey to configure the survey-related settings.
